New Patient Registration

non_nhs_services3You may register as a patient by calling at your surgery where you will be asked to complete a new patient questionnaire. You also will be asked to provide proof of your identity and place of residence. This will be in the form of photo ID, usually your passport and a utility bill. Patient Services staff will ask for your permission to photo copy the documents for our records.

If you are currently on repeat medication you are advised to make an appointment with a doctor or nurse. Omitting to make an appointment could result in a delay in providing you repeat prescriptions. 

As part of the normal registration process you will be registered as a patient of the practice. You do, however, have the opportunity to state a preference to receive services from a specific practitioner. Where this request is made we will endeavour to comply with your request, however, we do withhold the right to refuse your request if we have reasonable grounds to do so.
