Repeat Prescriptions - Select Telephone Option 4

pill_bottleRequests for repeat prescriptions can be made by completing a Prescription Request Form available at the surgery or complete the retained part of your computerised prescription and return it to the surgery.   You may also wish to order via your pharmacy if they collect from us - please check with your pharmacy to ensure that this service is available from them - Or from our Website – www.leacroft.co.uk   Please give at least 2 working days notice before collecting your medication.

Patients requiring medication to take on holiday or trips for business abroad, will be provided up to a period of 3 months. Medication required beyond 3 months must be obtained from a local source in the country in which you are visiting. Please ensure you have sufficient medication for your trip as in these circumstances, we cannot provide prescriptions to a 3rd party, for example a member of your family to deliver to you whilst abroad.

It is possible to avoid a further visit to the surgery by collecting your completed prescription direct from a local Chemist – please ask patient services staff for further details. Please note that if you request medication that has not been given to you on a regular basis the doctor may need to see you.

Patients who are prescribed long term medication will be subject to a regular medication review by a doctor. Medication reviews can be made either on the telephone with a doctor, for your convenience or by doctor consultation.  Please take note of any advice provided to you by patient services staff, by notification on the retained part of your prescription or by direct letter. It is in the interest of you our patient, that such reviews are regularly maintained to ensure you receive the most appropriate medication for your needs.
