Introducing the Leacroft Community Hub

The Leacroft Community Hub is our new initiative to address patient social needs.

It is made up of a team of Practice Health Champions (PHC’s for short!) who are Leacroft patients that volunteer their time, ideas, and enthusiasm to organise and run activities and sessions with a view to:

  • Meet the social requirements of our patients by providing the support and connection people need to improve their own health
  • Enable Leacroft and patients to find new ways of working together; make a difference in our patient’s lives and improve services through social interaction.

You may have seen our Practice Health Champions outside the practice this week introducing themselves to patients. They will return next week for more meeting and greeting so if you haven’t had the chance to pop down please come and say hello.

The PHCs will be hosting:

☕Monthly coffee mornings – starting 23rd August 10 am – 11.30 am for patients in the Leacroft meeting room. We are restricted on the number of attendees and therefore this will be invite only, if you are interested please let the reception team know to add you to our waiting list. You will then be contacted prior to the coffee morning by one of the team to confirm your booking.

🚶‍♂️🚶‍♀️Leacroft weekly health walk from the practice – Thursday’s at 10am open to all patients.

Please take a moment to fill out our Mailchimp survey here (this should take less than 5 minutes) to help us establish the needs of our patients and what activities would be the most beneficial for our PHCs to run. All sessions will be advertised on the waiting room notice boards and via patient newsletters.
